Sign up today with our secure online portal!
Family members can be added when you enroll, or at any time after. We accept all major credit/debit cards, most HSA and FSA cards, and you'll always have access to a receipt. Sunnybrook Wellness Plans have a one-time activation fee to set up monthly payments.
Your membership is effective immediately. Simply call our office to schedule an appointment when you are ready!
Please note, treatment plans are based on clinical findings at the time of your exam. If additional conditions are identified - such as periodontal (gum) disease - your recommended treatment plan and fees may be adjusted accordingly.
